New Patient Instructions

Welcome to Almadina Medical Clinic. To become a new patient, please complete the online registration form or visit the clinic in person with your Alberta Health Care card and photo identification.

  • Complete the new patient intake form before your first visit.
  • Bring a list of current medications, allergies, and past medical conditions.
  • Arrive 15 minutes early to verify your information.
  • Minors must be accompanied by a parent or legal guardian.

What to Bring to Your Visit

  • Valid Alberta Health Care card (or out-of-province coverage details).
  • Photo identification.
  • A current list of medications, vitamins, and supplements.
  • Any recent lab results, imaging reports, or specialist letters relevant to your visit.
  • Insurance or WCB information, if applicable.
  • The form you need completed, if your visit is for form completion.

Alberta Health Care / Insurance

Almadina Medical Clinic bills Alberta Health Care directly for insured services. Please present your valid Alberta Health Care card at every visit.

Uninsured services — such as some form completions, driver medicals, sick notes for employers, and cosmetic procedures — may require direct payment. We accept debit and major credit cards. If you have private insurance, please check with your provider for reimbursement options.

Appointment Policy

Appointments can be booked online through our booking page or by calling the clinic during business hours. We recommend booking in advance for family doctor visits, follow-ups, and form completions.

  • Please arrive on time. Late arrivals may be rescheduled.
  • One appointment per patient, per concern, to ensure adequate time.
  • If you are running late, call the clinic as soon as possible.

Cancellation & Missed Appointment Policy

We understand that plans change. If you need to cancel or reschedule, please give us at least 24 hours notice so we can offer the spot to another patient.

Repeated missed appointments or late cancellations may result in a fee or restriction on future online booking. If you miss an appointment, call the clinic to rebook.

Walk-In Policy

Our walk-in clinic operates on a first-come, first-served basis based on doctor availability. You can check live walk-in status and join the queue online before arriving.

  • Walk-in is for non-emergency concerns only.
  • Bring your Alberta Health Care card and medication list.
  • Wait times are estimates and may change based on patient volume and complexity.
  • Joining the queue does not guarantee same-day assessment if capacity is reached.

Prescription Refill Policy

Prescription refill requests should be made at least 48 hours before you run out of medication. You can request a refill by calling the clinic or booking a short appointment if a medication review is due.

  • Narcotics and controlled substances require an in-person visit and cannot be refilled by phone.
  • New medications or dosage changes always require an appointment.
  • Pharmacies cannot automatically refill prescriptions without authorization from your doctor.

Test Results Policy

Most test results are reviewed by your doctor and communicated to you within a few business days. If your results require follow-up, the clinic will contact you to book an appointment.

  • Do not assume no news means normal results. If you have not heard back within one week, call the clinic.
  • Some results are discussed in person rather than over the phone for your privacy and safety.
  • We do not email detailed medical results to protect your privacy.

Form Completion Policy

Medical forms for employment, insurance, travel, disability, and licensing require an appointment. Bring the blank form and any supporting documents to your visit.

  • Some forms require a physical exam and cannot be completed remotely.
  • Allow up to two weeks for complex forms requiring records review.
  • Uninsured form fees may apply. Ask the clinic for details before your appointment.

Privacy & Online Form Safety

Protecting your personal health information is our priority. Our online forms and booking system use secure connections, and your data is handled in accordance with Alberta privacy legislation.

Important: Do not submit highly sensitive medical details — such as detailed symptoms, mental health crises, or explicit health histories — through general online forms or email. For personal health concerns, book an appointment or call the clinic directly.
